1. Sponsor

The Project Otter Name Vote and Gift Card Drawing is sponsored by PriceMDs.com, 19321 US 19 North, Building C, Suite 509, Clearwater, Florida 33764 (“Sponsor”).

2. Eligibility

The gift-card drawing is open to legal residents of the United States who are 18 years of age or older at the time of entry.

PriceMDs employees and contractors, and members of their immediate families or households, may participate in the mascot-name vote but are not eligible to receive the gift-card prize.

3. Campaign Period

Voting opens on October 5, 2026, at 12:00 a.m. Eastern Time and closes on October 23, 2026, at 11:59 p.m. Eastern Time.

PriceMDs’ website and submission records will serve as the official timekeeping method for the campaign.

4. How to Vote and Enter

To participate, visit the Project Otter voting page during the campaign period, select one proposed mascot name, and submit the requested information through the official voting form.

The proposed names are:

• Dr. Otter
• C.C. (Cost Containment) Otter
• Ollie Otter

Full name and email address are required. Company information is optional. No purchase or payment is required.

Only one valid vote and one gift-card drawing entry are permitted per email address. If more than one submission is received from the same email address, the first valid submission will stand.

Incomplete, automated, fraudulent, manipulated, or otherwise invalid submissions may be removed at the Sponsor’s discretion.

5. Mascot Name Selection

The proposed mascot name receiving the highest number of valid votes at the close of the campaign will become the official PriceMDs otter name.

If two or more names receive the same highest number of valid votes, PriceMDs will select the official name from among the tied options.

The winning otter name is scheduled to be announced on Monday, October 26, 2026.

6. Prize and Odds

One eligible participant will receive one $100 digital Visa or Mastercard gift card. The approximate retail value of the prize is $100.

The gift-card drawing is separate from the mascot-name vote. The name selected by a participant does not affect that participant’s chances of receiving the prize.

The potential prize winner will be selected at random from all eligible, valid entries. Odds of winning depend on the total number of eligible entries received.

The prize is non-transferable. PriceMDs may substitute a prize of equal or greater value if the stated prize becomes unavailable.

7. Prize-Winner Selection

On or about Monday, October 26, 2026, PriceMDs will randomly select one potential gift-card winner from all eligible entries received during the campaign period.

Selection as a potential winner is subject to verification of eligibility and compliance with these Official Rules.

8. Winner Notification and Verification

The potential gift-card winner will be notified using the email address submitted with the entry on or about Monday, October 26, 2026.

The potential winner must respond within five business days of the notification and may be required to confirm eligibility and provide information reasonably necessary to deliver the prize.

If the potential winner does not respond within five business days, cannot be contacted, is found to be ineligible, or does not comply with these Official Rules, PriceMDs may select an alternate potential winner.

The gift-card recipient will not be publicly identified without that person’s permission.

9. General Conditions

PriceMDs reserves the right to disqualify any participant who interferes with the voting process, submits false information, attempts to manipulate the results, or otherwise violates these Official Rules.

If fraud, technical failure, unauthorized intervention, or another issue materially affects the integrity or proper operation of the campaign, PriceMDs may suspend, modify, or terminate the campaign and determine the results using a fair method based on the valid submissions received.

PriceMDs is not responsible for lost, delayed, incomplete, misdirected, unreadable, or unsuccessful submissions caused by technical, internet, website, email, device, or service-provider issues outside its reasonable control.

10. Release and Limitation of Liability

To the extent permitted by law, participants release PriceMDs and its officers, employees, contractors, agents, and service providers from claims arising from participation in the campaign or the acceptance, possession, use, or misuse of the prize.

Nothing in these Official Rules excludes rights or liabilities that cannot lawfully be excluded.

11. Governing Law

These Official Rules and the campaign will be governed by the laws of the State of Florida, without regard to conflict-of-law principles, except where prohibited by applicable law.

PriceMDs may collect:

• Your full name
• Your email address
• Your company, if provided
• Your selected otter name
• Your consent and communication preferences
• Submission and technical information reasonably necessary to operate, secure, and validate the voting process

2. How the Information Is Used

PriceMDs may use the information to:

• Record and validate your vote
• Enforce the one-vote-per-email rule
• Administer the mascot-naming campaign and gift-card drawing
• Verify eligibility and contact the potential prize winner
• Deliver the gift-card prize
• Send the winning-name announcement and additional PriceMDs information if you selected the optional email-updates checkbox
• Protect the campaign against abuse, manipulation, or technical problems
• Meet applicable legal and recordkeeping obligations

3. Optional Email Communications

Agreeing to receive email updates is optional and is not required to vote or enter the gift-card drawing.

If you select the optional email-updates checkbox, PriceMDs may send you the winning otter name and additional information about its services. You may unsubscribe from those communications at any time using the unsubscribe instructions provided in the email.

4. Sharing of Information

PriceMDs may share campaign information with service providers and systems used to operate the campaign, including website hosting, form processing, CRM, email, security, and prize-fulfillment providers.

PriceMDs may also disclose information when required by law or when reasonably necessary to protect its rights, systems, users, or the integrity of the campaign.

PriceMDs does not sell personal information collected through the Project Otter voting form.

5. Prize-Recipient Information

The potential prize recipient will be contacted privately by email. PriceMDs will not publicly identify the prize recipient without that person’s permission.

6. Data Retention

Campaign information will be retained for as long as reasonably necessary to administer the vote and drawing, deliver the prize, resolve questions or disputes, maintain appropriate records, and satisfy applicable legal obligations.
